Prepare For Your Child’s First Pediatric Dentist Visit

During your first visit to ABC Dentistry for Children in Gilbert, you and your child will build a vital foundation that starts your relationship with our Arizona pediatric dentistry team. If your child is at least one year old, it’s recommended that you bring them to our child-friendly office to review their medical history and go over oral hygiene routines.

 

To help prepare for your first visit to ABC Dentistry for Children as a new patient, we have included some forms that you can print and fill out prior to arrival. You can fill out the New Patient Form and HIPPA Policy form ahead of time, though we will always have a blank copy for you if you would like to fill it out when you arrive.

If you have someone that will be bringing your children to their appointment on a regular basis, you may want to have the below form notarized. You can also sign it at our office if you do not want to get it notarized.
